Program Requirements for New Clients at Nirvana
Anti Aging, Rejuvenation, and Natural Healing Programs
For those clients who are seeking a comprehensive integrative program, you will begin your journey with a thorough health evaluation with one of our associate Integrative Medical Doctors that will seek to discover the root cause of any major or minor imbalances that are preventing you from reaching your optimum.
Fitness with Oxygen Programs
For those clients who are coming to Nirvana to participate in our Fitness with Oxygen Programs, we require that you do have a health and fitness evaluation with our fitness therapist and a medical intake with our one of our associate medical doctors to participate in the program.
Outside Doctor Referrals
For those clients who are referrals to Nirvana from MDs, DOs, and DDs, you are required to bring a medical prescription from their office or have them fax it to 805-693-8100 at or before the time of your first treatment oxygen treatment, HBOT or EWO2.

Please complete and bring with you the First Time Client paperwork to your initial consultation, which provides our program director and associate medical doctor with vital information necessary to prepare your personalized plan at Nirvana.
Schedule an Appointment with Gary Foresman, MD, and Middle Path Medicine at Nirvana
New Patients: Call 805-693-8100 to schedule your first appointment with Dr. Foresman. Each patient is required to complete a Medical History Package and to see the Medical Assistant for a Wellness Evaluation before seeing Dr. Foresman for your initial one hour appointment. In preparation for your first appointment, may we also suggest that you read through Dr. Foresman's Your Journey to Health and Healing and his Foundations of Health.
